健康检查计划条款 Terms and Conditions:

客户收到由MedDx寄出之确认成功付款电邮后，

本身体检查计划有效期为三个月，客户必须于三个月内(由确认付款日期起计)接受有关检查，逾期作废。

The plan will be valid for 3 months starting from the date of payment.

进行健康检查后，一般情况下，可于三至十个工作天跟进检查报告。

The health check report would be issued around 3-10 working days normally after the check-up.

客户要亲身往检查中心领取报告

Report should collect in person

订购一经确认，不设退款。

Amendment or cancellation is not allowed once the payment is confirmed, and is not transferable and refundable.

如有争议，Meddx保留最后决定权。

In case of disputes, the decision of MedDx shall be final.

所有身体检查并非作为医务诊断或治疗用途。

All tests are not for the purpose of medical diagnosis or treatment.
